A Skilled Nurse is directly responsible for assessing, planning, providing, and documenting nursing care.

Home Health Aide

Home Health Aides assist patients in their home with their activities of daily living.

Home Health Aides provide assistance with:

  • Personal Care – Shower, Bath Chair, Bed Bath, etc.
  • Dressing
  • Grooming
  • Mouth Care
  • Ambulation/Transfers
  • Position Changes
  • Meal Preparation, Meal Set-up, Serving, and Feeding.
  • Medication pick up
  • Grocery Shopping
  • Light housekeeping – Sweeping, dishes, vacuum, etc.
  • Change linens/Make bed
  • Laundry

Physical Therapy

Therapy is for the preservation, enhancement, or restoration of movement and physical function, impaired or threatened by disease, injury, or disability.

Therapy treatment utilizes therapeutic exercise, physical modalities (such as massage), assistive devices, and patient education and training.

Therapy can also help patient’s obtain assistive devices and other necessary mobility equipment needed for daily activities.

Physical Therapy can help reduce pain, increase strength, and balance to encourage safety and independence.
